Symptom narrative

When the Pentacel was prepared for administration, it was reconstituted with the diluent for a Proquad/MMR/Varicella vaccine. The diluent that comes in the Pentacel package, containing the Dtap-IPV portion of the vaccine, was not used or administered. The patient's parent has not reported any adverse symptoms or effects.
