Symptom narrative

Vaccines given to the incorrect patient, meant for the sibling. For Rotarix the possible common side effects of loose stools and loss of appetite. Advised the mother to bring the patient to the ED if patient has abdominal pain, vomiting or blood in the stool. For PCV 20, common possible side effects fever, body aches, pain at injection site. Advised ED precautions if there are anaphylactic reactions.
