Symptom narrative

Patient says that he received the vaccine around 10:30 in the morning and began to have significant light-headedness, dizziness, nausea twelve hours later. He said the he developed vertigo and brain fog that was an ongoing problem for the next two years, and he worked with a neurologist to try and correct it. After two years the problem subsided.
