Symptom narrative

The first dose of MMR and varicella was administered to the minor before having evidence form the storage unit refrigerated temperature logtag (03/21/2026 to 03/24/2026 period time). The mother of the minor was called and told about the situation, she said that she understood but refuse the administration of a second dose. To benefit the patient a additional dose was offered.
