Symptom narrative

A dose of MMR and varicella vaccine was administered to a minor; however, there was no record of freezer temperatures from the Log Tag device between March 21, 2026, and March 24, 2026. The cause is unknown. The minor's mother was contacted; she stated she understood the situation and declined the administration of a second dose of the vaccines. In the patient's best interest, the additional dose was offered, and the Log Tag device was repaired.
