Symptom narrative

Patient reported that 2 days after getting COVID vaccine, she noticed she had slurred speech. She went to the hospital and was diagnosed with 'mini-stroke'. She was hospitalized for 3 days. And because of that she has decided to not get COVID vaccine anymore.
