administered boostrix and the intended vaccine was flu vaccine; incorrect vaccine to 3.5 year old; This non-serious case was reported by a physician via call center representative and described the occurrence of wrong vaccine administered in a 3-year-old male patient who received DTPa (Reduced antigen) (Boostrix) for prophylaxis. Co-suspect products included Flu unspecified (Flu vaccine) for prophylaxis. On 17-MAR-2026, the patient received Boostrix. The patient did not receive Flu vaccine. On 17-MAR-2026, an unknown time after receiving Boostrix, the patient experienced wrong vaccine administered (Verbatim: administered boostrix and the intended vaccine was flu vaccine) and inappropriate age at vaccine administration (Verbatim: incorrect vaccine to 3.5 year old). The outcome of the wrong vaccine administered and inappropriate age at vaccine administration were not applicable. Additional Information: GSK receipt date:18-MAR-2026 The patient was 3.5 years. It was the wrong patient and wrong vaccine. The batch number was not provided upon follow up with the reporter.
