pcv 15 was given when hep b was requested. when appt was made reason for visited was entered as pcv 15 vaccines visit instead of hep b. when giving patient vaccine i stated to mom hold leg because pcv 15 burns, she never refused. Dr realized wrong vaccine was given when she billed a few hours later. mom was called and a message was left, mother called back with father on line later that day so we could explain,
