Symptom narrative

Swelling, pruritus, erythema, warmth to injection site 2 days after injection of DTaP to left deltoid. 5th dose of DTap no hx of rxn to vaccine in the past. On exam 11x12cm circular area of erythema and warmth with slight swelling to left deltoid. No tenderness, fluctuance, vesicles, streaking, papules, petechiae. FROMI. NVI. Motrin and Benedryl pm.
