Symptom narrative

Pt came in to the pharmacy on 3/23 and stated that she believed her Tdap vaccine was given in the bursa area of the arm and that's why after a month it is still hurting her. She originally disclosed this information to my technician and wanted it to be a teaching moment to my staff so that it's not repeated again for future for others. I later followed up with the patient and she stated that since she was still having trouble with her arm, she went to get it checked out with dr. Dr suggested maybe due to injection in bursa area. She then went into that she is seeing PT and getting regular massages for the pain and that she doesn't have full motion with that arm. This then got escalated to my supervisor.
