I grabbed the flu shot out of our fridge but it wasn't tight in the box but for some reason did not double check the sticker to age, I then put the sticker on the paper and asked patient if any allergies, issues with flu shot in the past, any infections and so on, and when he stated no to call questions I handed him the paper to sign consent to receive the injection. I then proceed with administering the injection and finished with the rest of his check out and told him to slowly leave. I then went to put the information into the computer and couldn't figure out why it wouldn't take my lot number and then that is when I realized it was wrong dose. I then informed my physician I work with and then I informed my supervisor and asked the steps I should do on this. I then notified the patient, and his response was "ok" I then told him how I will continue with my documentation and informing and correcting and he stated "ok, not a problem" He had no reports of any issues at that time from the vaccination
