Symptom narrative

patient went to Urgent Care on 3/9/26 for sore throat and mentioned that she has been having pain at injection site since 2/18/26 - there is a small lump at site - it is painful to lie on it - she has not taken any medication for it - able to move all extremities written in report - mom called office on 3/10/26 at 4:34 and left message - office called mom 3/11/26 at 8:30 and left msg for call back - mom called back to the office on 3/12/26 and states she has a bump sticking out of her arm - fingers are numb and she is having so much pain- denies any medication for pain- appt scheduled for 3/12/26 - seen by oncall provider in office - she c/o tingling over ther 3rd/4th finger and difficulty lifting her hand over her head - she was referred to orthopedic surgeon on 3/18/26 - she had normal sensation to light touch to medial, ulnar, radial, and axillary nerves -intact deltoid function - xray done and showed scar tissue - ordered Physical therapy
