Symptom narrative

Incorrect dosage of vaccine was administered. Was given 0.25 mL- dosage for 6 months through 11 years- patient was 18 at time of vaccine administration. No signs or symptoms of adverse effects seen.

Cyclical vomiting syndrome; PCOS; Migraines

Other medications

Vitamin D 1250 mcg weekly. Epinephrine auto injector for use as needed for anaphylaxis
