Symptom narrative

13:30 14yr old male patient ambulated into the exam room with mom and sister, Pt. alert/orientated, able to make eye contact, answer questions. Informed patient and mother of vaccine gave VIS to mother. Pt. and mother understood vaccine. Pt. would like vaccine on left deltoid. cleaned left deltoid with alcohol, gave prefilled HPV vaccines with 25g, 5/8-inch needle in left upper deltoid, band aid placed. Pt. tolerated without incident. Pt. walked across the room to pick up a sucker, pt came back sat down in chair. Visiting with patient and mother. Pt. became pale, quit talking and stated he was hot and dizzy. Encourage pt to stay sitting. Pt verbalized understanding gave verbal support to patient and mom. 13:34 pt. became pale, sweating, layed head down on the counter. BP at 13:34 68/37, pulse slow steady, ammonia salts given, orange juice on hand, cold rag placed on neck. pt. resting with eyes closed with mom by his side. BP at 13:40 80/47, resting with eye closed, pt able to answer yes/no questions, 13:43 BP: 103/63, more alert, small sip of juice given, pulse slow steady, 13:47 BP 100/60, alert, oriented answering questions, stood up with no dizziness,13:50 discharged with mom. Pt. alert oriented.
