Symptom narrative

Patient was crying & upset prior to vaccines given. Also c/o some skin itchiness without rash. 5-10 after vaccines were given, he developed hives on Rt arm at elbow & forearm. Also hives around Rt eye. Monitored & hives faded after 20 minutes without treatment.
