Per DNP, FNP - Witnessed approximately 1 minute tonic clonic convulsive syncope episode, pt was pale and unconscious. Pulse ox 98%, pulse 110 and quickly down to baseline 88 within 2 minutes, color improved, and talking. No incontinence. Pt did not hit her head, mother present and was able to gently lay her down on table during episode and called for help. 911 was called and patient was taken to the hospital by ambulance.
