Patient is a 27 year old pregnant with her 3rd child in her second trimester with a due date of July 6, 2026. She arrived for her routine prenatal visit and when asked by the provider if she wanted a flu shot she replied, "yes." I then received a message from the provider to get the vaccination ready. I then went into the room and gave the injection while the provider continued speaking to patient. Once I return to my desk to input the vaccination details I noticed she had already received a flu vaccine in January. I advised the provider, as well as, my office manager. Provider did inform patient over the phone at the end of the day of our mistake.
