The suspect product included Pre-filled Syringe injection (batch no. 3030342) for COVID-19 prophylaxis. Concurrent medical conditions included Lung disease, Hypothyroidism (pretty out of the box type. It was not a norm), Diabetes (out of compliance) and Blood pressure high (out of compliance). On 19-Sep-2023, the patient received seventh dose of mRNA-1273.815 (SPIKEVAX 2023-2024 PFS) (unknown route) .5 milliliter. On an unknown date, the patient started Pre-filled Syringe (unknown route) at an unspecified dose. On an unknown date, the patient experienced GENERAL PHYSICAL HEALTH DETERIORATION (multiple medical issues/body is more like 95 years), VOCAL CORD DISORDER (had damaged vocal cords) and DYSPHONIA (going to lose voice/cannot speak/losing voice talking). At the time of the report, GENERAL PHYSICAL HEALTH DETERIORATION (multiple medical issues/body is more like 95 years), VOCAL CORD DISORDER (had damaged vocal cords) and DYSPHONIA (going to lose voice/cannot speak/losing voice talking) had not resolved. Patient had existing medical conditions before, she got the shot early on because of these conditions. Patient took unspecified medications for diabetes, high blood pressure and got very unusual medicine for hypothyroidism. Patient was not taking any medication for diabetes at this time because had another underlying condition that was interfering and was being exacerbated by the medication called hypothyroidism. The previous doses were reported as dose 1, dose 2, dose 3 and dose 4 of the Moderna COVID 19 Vaccine, administered on 11-Mar-2021, 08-Apr-2021, 24-Aug-2021 and 22-Mar-2022, with batch numbers 038A21A, 032B21A, 002F21A and 048L21A, respectively. Dose 5 and Dose 6 (Moderna COVID 19 Vaccine, Bivalent [Original and Omicron BA.4/BA.5]) were administered on 28-Sep-2022 and 05-May-2023, with batch numbers AS7148B and 069422A, respectively. It was reported that, the patient had multiple medical issues and received them very early on, even though she was not 65 at the time, her body was more like 95. But her very first shot was Moderna, and she stayed with Moderna all the way. Patient put her glasses on for looked up her vaccine records. It was added; she had been taking Covid vaccine religiously every 6 months since she got it. One year she thought she had three, reason not sure and had damaged vocal cords. She was going to lose voice. She could not speak, losing her voice talking, and was trying to calm down because it took a lot of effort to get her vocal cords out.
