Symptom narrative

Patient was to receive Pentacel vaccine, vial 1 / 2 was not used to reconstitue vial 2/2. The 2/2 Vial was reconstituted with Sterile Dilutent water instead of the DTaP-IPV component. Patient was only given the Act HIB without the DTap-IPV component. This vaccine was given 3/26/26. Contacted the manufacturing company Sanofi Pasteur and was told that the live vaccine was not administered and dose should be repeated per Ada call reference # 02853621
