Patient came into the pharmacy requesting a second dose of the MMR vaccine. Pharmacy manager attempted to verify vaccine history of the patient and was able to find one dose in the pharmacy system given on 9/7/25, but was not able to further access more information from the state immunization registry due to access being forbidden. Patient and pharmacy manager had a discussion about a conversation that the patient and another pharmacist had recently and patient verbalized the first dose being given on 9/7/25. The next day, corporate sent an email inquiring on a dose of MMR being given and it was shown that the patient received a dose of MMR vaccine on 10/5/19 at another pharmacy.
