Site: Pain at Injection Site-Mild, Systemic: No reaction-Mild, Additional Details: the Shingrix vaccine was inadvertently documented as a prefilled syringe formulation (GSK Shingrix 0.5ml Recombinant IM; Without Preservative). A reconstituted formulation (GSK Shingrix 0.5ml Recombinant IM; Without Preservative) was actually administered.
