Symptom narrative

Tdap and Hep B given L Deltoid a few inches apart. On 3/28/2026 noticed redness/warmth/tenderness to left upper arm. Site red and warm to touch with noted red circle approx. 3 inches in diameter. OTC allergy medicines given and ice pack to placed to LUE. Employee to report continued or worsening symptoms and will refer to Occupational Med if continues.
