Symptom narrative

Client recieved TDAP vaccine. Client tolerated vaccine well. Client was sitting in a chair with the rest of his classmates for a few minutes after getting the vaccine to monitor for any reactions. A classmate of client said client leaned over on her and she thought he was just being fun so she did not do anything. The school nurse saw client leaning and she went to check on him. She noticed he was pale and she helped him to the floor. School nurse reports clients eyes rolled back and was unresponsive but breathing . School nurse reports thise occured for no more than 10 seconds and client came to on his own. Client got up with the help of the school nurse and walked with her to the office where they called his parents
