Symptom narrative

It looks like the vaccine was given in the incorrect location. Patient claims it was SQ, but it was definitely not in the Deltoid muscle. Hard, red, local reaction

Lab data

Patient has just been applying ice to the affected area. Redness and irritation have decreased from initial occurrence but is still pronounced.
