Patient received a vaccine not meant for his age. Patient recently came to [withheld] from [withheld] and had a large amount of vaccines still needed. The provider thought it was ok to give the combination vaccine to the patient since he needed all 4 vaccines (Dtap, Hib, Polio and Hep B) and several others.
