Provider ordered TDAP vaccine for pt that is 28w5d, HPV given in error. Provider notified and states no adverse affect to pregnancy, mother and fetus. Provider discussed with pt. Patient returned 3/31/26 to receive TDap, no noted effects from incorrect vaccine from prior visit. **Documentation was not done for lot or expiration.
