Symptom narrative

Patient had a site reaction after HPV vaccine was administered. The injection site swelled and became hard. I went and got PA-C to examine the injection site. Patient stated that sometimes injection site reactions happen, she asked Patient to stay a few minutes so that we could monitor her and make sure she did not have any other reactions. Patient stayed for 15 minutes with no other reactions noted. She was told to monitor it and if she develops any other reaction symptoms to go to the ER. She was also told that she can ice the area and take either ibuprofen or tylenol as needed. Patient voiced understanding and left the clinic in stable condition.
