Product preparation issue

Symptom narrative

Patient received only the liquid component of the Menveo pediatric vaccine. Menveo requires reconstitution of two separate vials (liquid and powder components) prior to administration; however, only one vial (the liquid component) was administered. The second (powder) component was not given. This was an administration error. The patient did not receive a properly prepared dose of the vaccine.
