Symptom narrative

Approximately 15 minutes post vaccine administration child's right arm and right thigh noted with flat light red discoloration. Child stayed in clinic 30 minutes after vaccines administered with no further symptoms. Redness become lighter. Nurse contacted child's mom the next day and she reported the child did fine and the redness disappeared.
