Symptom narrative

Patient received Proquad/Quadracel on 9/29/25 when here for nurse visit - he then received them again at his 4 year pe done at age 4.5years - Hep A #2 had been given at 5 months so it was repeated at the 4 year visit to be UTD - family was notified - child had some swelling in his right arm with pain down into his hand after vaccines - given Tylenol - seen in office on 4/1/26 - full ROM for arm and bilateral strength good in both hand
