Symptom narrative

Provider placed order for Gardasil 9 vaccine. After obtaining consent and verifying patient's name DOB and allergies, medical assistant proceeded to administer vaccine. Upon administering vaccine Medical assistant realized she was holding a Tdap vaccine empty syringe instead of Gardasil (HPV). Immediately she notified provider and brought patient back to room. Provider assessed patient for signs/symptoms and Tdap status, negative findings. New order was placed for Tdap and consent was signed by patient. Gardasil 9 vaccine administered as previously ordered.
