Symptom narrative

Patient presented to the office 4/1/2026 for a progressively spreading erythematous rash approximately 8 cm x 10 cm on the right thigh. Patient's mother reports that he received his 4-year vaccinations Quadracel and Proquad on 03/30/2026. Since then, he has been experiencing a progressive spread of a rash on his right thigh. Provider advised to take Benadryl and Keflex as prescribed. Patient advised to follow up in office in 48 hrs , Sooner if worse or other symptoms
