Expired dose given; Wrong age for first dose; This non-serious case was reported by a other health professional via call center representative and described the occurrence of expired vaccine used in a 15-month-old male patient who received Hib (Hiberix) (batch number 3PY4Y, expiry date 17-MAR-2026) and (batch number 9AT95, expiry date 17-MAR-2026) for prophylaxis. On 24-MAR-2026, the patient received the 1st dose of Hiberix. On 24-MAR-2026, an unknown time after receiving Hiberix, the patient experienced expired vaccine used (Verbatim: Expired dose given) and inappropriate age at vaccine administration (Verbatim: Wrong age for first dose). The outcome of the expired vaccine used and inappropriate age at vaccine administration were not applicable. Additional Information: GSK Receipt Date: 25-MAR-2026 The health care professional reported that a patient was accidentally given an expired dose of Hiberix. The vaccine administration facility was the same as primary reporter.
