Symptom narrative

Patient had come into pharmacy to receive Pneumonia vaccine, and upon reviewing her immunization history in our Immunization Registry, patient advised to get Covid vaccine with last dose showing received on 9/23/25. Patient agreed and received dose. Husband was grocery shopping while patient received both vaccines and stopped by to verify which vaccines she received stating she was to only receive the Pneumonia vaccine and she had just left the doctor's office and had already received the Covid vaccine that same day. We immediately contacted the office and they verified the following day that she did receive the Covid vaccine.
