SHINGRIX packaging of PFS color being the same as Havarix; This non-serious case was reported by a other health professional and described the occurrence of circumstance or information capable of leading to medication error in a patient who received Herpes zoster (Shingrix) for prophylaxis. On an unknown date, the patient received Shingrix. On an unknown date, an unknown time after receiving Shingrix, the patient experienced circumstance or information capable of leading to medication error (Verbatim: SHINGRIX packaging of PFS color being the same as Havarix). Additional Information: GSK Receipt Date: 24-MAR-2026 The health care professional reported that finishing up a breakfast in this morning and wanted to forward customer feedback with the Shingrix packaging of prefilled syringe color being the same as Havrix. They were making sure to tell the staff to double and triple check before giving. They checked the fridge and its properly label and the vaccines are separated. The batch number was not provided and we and unable to contact the reporter.
