Pt recently received JEV (Lot number JEV24A173) and anthrax vaccine (Lot # 300223A) on 2/27/26. TPt reports both vaccines given in left arm. This was patient?s 2nd dose of anthrax, 1st dose of JEV. Pt reports some nausea, tunnel vision, limb ?heaviness?. Pt does report hx of presyncopal episodes outside of immunizations. She notified the technician and reports she was sat down. States she felt shaky, had difficulty verbalizing and reported possible bradycardia. She was given water. Within 10-15 minutes she reported improvement in symptoms. Was able to ambulate and was discharged from the immunizations clinic. Within 30 minutes all symptoms were resolved. She reports towards the end of the day developed sore throat/?scratchiness?. No angioedema, shortness of breath. No urticaria. Over the weekend reports worsening of sore throat. On Monday (3 days post immunizations) pt awoke with hoarseness, body aches, subjective fevers. Due to persistent complaints she was evaluated in the ER on 3/4/26 and diagnosed with suspected viral pharyngitis vs. laryngitis vs URI. She was given dexamethasone by mouth for symptoms and advised to continue supportive care. On 3/6/26 she presented to the clinic reporting ?spreading rash? down from her left arm injection site as well as worsening nasal congestion. Due to suspected cellulitis she was treated with Keflex x 10 days. Seen again in clinic on 3/9 with improvement in rash and symptoms told to complete antibiotic course. Pt reports sore throat, nasal congestion, body aches improved as well.
