Symptom narrative

Following the last injection, client asks for water, becomes pale, and losses consciousness causing him to slump towards the right in the chair. Nurse calls for office manager to bring vital machine. Client continues to breathe, he was assisted to a reclined position in the chair with his legs elevated and head lowered, airway patent. Only unconscious for a few seconds to a minute, would answer yes and no questions. 1018- unconscious. 1023-first set of vitals obtained. BP 91/55 SPO2 98% and pulse was 91. Client was pale and skin clammy. Office manager and mother present. 1030-Cool washcloth applied to head and neck. Water and grapes provided as client was alert. 1035- BP 84/56 client alert, eating, drinking and talking with mother and staff. 1038- BP 90/55 pulse 85 and SPO2 97% room air. Alert, feeling better. 1040- Moves to standing position and tolerates. Stable. Leaves with parent. BP cuff- right arm, reclined position.
