Adverse Event Description Patient received MenQuadfi and Bexsero vaccines. Approximately 1 minute after vaccine administration, while seated, the patient verbalized ?oh no? and acutely slumped from a seated position to a supine position. She was observed to have brief generalized seizure like movements. The movements were transient and consisted of jerking activity without subsequent post ictal state, making myoclonic jerks more likely than an epileptic seizure. She returned to baseline mental status immediately after the episode. During the event, blood pressure was noted to be transiently decreased. There were no signs of anaphylaxis, including no urticaria, angioedema, vomiting, wheezing, or respiratory distress. Oxygen saturation remained stable at 99 percent throughout the episode. The patient reported she had not eaten anything all day prior to vaccination and felt the episode may have been related to hunger. She was given oral juice, after which blood glucose measured 88 mg per dL. Blood pressure subsequently normalized. Electrocardiogram was obtained and was normal, with no QT prolongation or arrhythmia noted. The patient was observed for approximately 1 hour following the event without recurrence of symptoms and returned fully to baseline. Basic laboratory studies were ordered. She was discharged home in stable condition with plans for outpatient follow up.
