Symptom narrative

After administering the Tdap vaccine, the medical assistant stepped out of the room to allow the patient to get dressed. Upon checking on the patient the patient informed the medical assistant that she stepped off of the exam table and sat on the chair but woke up on the floor. Patient had a small lump on her forehead as well.
