Symptom narrative

Rash after her 4mo, 6mo, and 12 mo vaccines that appeared like "burned skin" with redness and papules. The rash appeared to cause discomfort and has left some hyperpigmentation to diaper area and upper thighs and subtle papular rash on labia.
