patient got a third shingrix shot after our facility recommended it. she received 3/24/26 in drs office then went back through her records to see she got in 2/2020 and again 5/2020 so she completed series and did not need another. however her original series from [withheld] did not report the first one so state registry only showed one which is why we recommended completing for her
