Symptom narrative

Patient was seen by APNP for a Diabetes Follow up visit, patient wanted the Covid and Influenza vaccine. The Covid vaccine was administered IM to the right deltoid, Influenza vaccine was administered IM left deltoid. Patient tolerated without complaint. Patient later sent a message to APNP indicating patient had bruising to the right upper arm. Patient denies fevers, warmth or redness at site of injection. Wife also indicates patient also had symptoms starting 03/11/26 at 4:30pm of not feeling well, shivering/shakes, no fever, lasted 3 days. Bruise started to appear after 5 days. Patient was taking Naproxen at the time of the injection.
