Symptom narrative

This nurse was seeing pt for an hemoglobin check. As efforts to complete care gaps, nurse noted pt due for MCV4 dose through System. Nurse offered vaccine and MOC agreed. After administration of vaccine and documentation, nurse logged on to State Database to provide parent with vaccine record. Upon review of vaccine record, nurse noted pt had been given first dose of MCV4 in 2023. Nurse immediately informed family and apologized. Informed MOC pt will need booster after turning 16. Upon refreshing system, nurse found first vaccine. Apologized again to family and gave precautions or reasons to call back. MOC verbalized understanding.
