Symptom narrative

Prior to vaccination, I asked patient if he has had any adverse events to vaccines in the past. He stated that he had once fainted after receiving the polio vaccine but had no reaction to his last Hepatitis B vaccine in 2022. I instructed the patient and his wife that I would monitor him following the vaccine. After the vaccine, patient sat in the waiting room with his wife and child. About 5 minutes later, the wife alerted me that he was feeling light headed and hot, so he wanted to sit in the vaccination room. We brought him into the room and I provided the wife with some paper to fan him. I asked the patient to sit on the floor in case he fainted, but he wanted to remain upright in the chair. His wife stayed with him and patient fainted in his seat, but remained upright. I asked his wife to help me move him to the floor so he could lay flat, but the patient came to and insisted on staying in his chair. I then called for a store manager to come check in on patient as well. Patient insisted he was feeling better and that this had happened once before. I told him I would like for him to stay seated and I would check on him and his wife every 5 minutes to make sure he was improving but if anything changed to let me know right away. After a total of 3 check-ins and 15 minutes, the patient said he was feeling a lot better and wanted to leave. I told the patient to let me know if there was anything else I could do for him and to let me know if he started to feel different once he stood up. Patient then left with his family.
