Symptom narrative

Patient was already in supine position due to history of syncope with blood draws. Patient stated that he was feeling fine after Tdap vaccine was administered. After HPV vaccine patient reported lightheadedness. Clinic RN was notified and involved in patient care. Legs were elevated, apple juice and water provided, ice packs applied to neck and forehead. Patient was continuously monitored by RN and this MA. Patient remained stable and was then discharged by RN in improved patient condition
