Symptom narrative

The YF-VAX was diluted with sterile water diluent rather than the sodium chloride diluent provided. CDC information states that if diluted with sterile water diluent, the vaccine is ineffective and should be readministered. The patient was contacted and opted to return for readministration within 24 hours. She reported no adverse effects.
