Symptom narrative

Pt received both immunizations on 4/1/26 and presented to the pharmacy on 4/4/26 stating her arm was hurting and that she had a fever of 102 and had been throwing up and having diarrhea. She stated she had received some "allergy shots" at her doctor's office on 4/1/26 before she received both the Boostrix and Twinrix on 4/1/26.
