On 20-MAR-2026, the patient received the 1st dose of Penmenvy. On 20-MAR-2026, an unknown time after receiving Penmenvy, the patient experienced inappropriate preparation of medication (Verbatim: administered only the diluent part of Penmenvy without adding the powder component) and inappropriate dose of vaccine administered (Verbatim: administered only the diluent part of Penmenvy without adding the powder component).
