A 72-year-old female patient received pneumococcal 20-val conj vac (dipht CRM197 protein) (PREVNAR 20), on 17Mar2026 as dose number unknown, single (Batch/Lot number: unknown), in right arm for immunisation. The patient's relevant medical history included: "Emphysema" (unspecified if ongoing); "Coronary artery disease" (unspecified if ongoing), notes: Coronary artery disease, coronary atherosclerosis; "coronary atherosclerosis" (unspecified if ongoing); "vaccination-site pruritus" (unspecified if ongoing). Concomitant medication(s) included: CYMBALTA, start date: 01Mar2026 (ongoing). Past drug history included: Tetracycline, reaction(s): "Allergies: Tetracycline"; Erythromycin, reaction(s): "Allergies: Erythromycin". Vaccination history included: Prevnar 13, administration date: 2019, for Immunization; Pneumovax, administration date: 2020, for Immunization. The following information was reported: ERYTHEMA (medically significant) with onset 17Mar2026, outcome "recovering", described as "Does not have hives but, still itching, occasionally, will turn red"; HYPERSENSITIVITY (medically significant), PRURITUS (medically significant) all with onset 17Mar2026, outcome "recovering" and all described as "That evening, started breaking out with hives, itching all over/Wondering how long those allergic reactions last."; URTICARIA (medically significant) with onset 17Mar2026, outcome "recovered" (2026), described as "That evening, started breaking out with hives, itching all over/Wondering how long those allergic reactions last.". Therapeutic measures were taken as a result of hypersensitivity, pruritus, urticaria, erythema. Clinical course: The patient asked how long to expect to experience itching after receiving PREVNAR 20 vaccine. She had vaccine on 17Mar2026 and started breaking out with hives, itching all over that evening. By next morning, she was sent to emergency room but was not admitted. She was given a steroid injection but had reaction to that. Then started on IV and was given Ativan. They gave her prescription for hydroxyzine, she was sent home with prescription, for 5-day course prednisone. Does not have hives but, still itching, occasionally, will turn red. Wondering how long those allergic reactions last. She is taking Benadryl because hydroxyzine made her sleepy and had upset stomach. Itching has gotten a little better, not as bad as it was at first but, most of times, occurs in late evenings. The information on the batch/lot number for pneumococcal 20-val conj vac (dipht CRM197 protein) will be requested and submitted if and when received.
