Site: Itching at Injection Site-Severe, Site: Pain at Injection Site-Mild, Site: Redness at Injection Site-Medium, Site: Swelling at Injection Site-Severe, Additional Details: Vaccine administered using standard deltoid landmarking. no immediate reaction observed. Patient tolerated procedure. Patient reports soreness evening of vaccination without redness. Redness noted the following morning after bandage removal. Progressive redness and itching over subsequent days. Findings consistent with a delayed local injection-site reaction following vaccination. Patient advises to monitor symptoms
